Then fill out the permit application and fax the permit application, insurance and map of the filming location (the map may be hand drawn) to: Lorraine Aguilar 213 977-8686.
The Forest Service requires a minimum of 72 BUSINESS HOURS to process a permit application and will not begin to process the permit until they receive the permit application, insurance and map.
INSURANCE REQUIREMENTS:
The Certificate of Insurance must add the following as additional insured:
U.S. GOVERNMENT USDA FOREST SERVICE 701 N. SANTA ANITA AVE. ARCADIA, CA 91006
AND the following clause MUST appear on the face of the Certificate of Insurance and as a clause or an endorsement in the insurance policy:
“IT IS UNDERSTOOD AND AGREED THAT THE UNITED STATES OF AMERICA, ANGELES NATIONAL FOREST, FOREST SERVICE , U.S. DEPT. OF AGRICULTURE, IS ADDITIONALLY INSURED SOLELY AS RESPECTS LIABILITY ARISING FROM OPERATIONS OF THE NAMED INSURED.”
PAYMENT OF FILMING FEES:
- Payment must be made prior to the first day of prep or filming by appointment only. An appointment date/time will be given to you once your application has been approved. A FilmL.A., Inc. representative may pay for, pick up and deliver your ANF permit to you for $150 plus the cost of the permit. Please request this service a minimum of 48 hours prior to your first film date.
- Payment must be made in the Forest Service office at: 12371 N. Little Tujunga Canyon Road San Fernando, CA 91342
- Payment must be made by check only. No cash. Make check payable to: Forest Service
